What is a GP Federation?

A GP Federation is a group of practices who have come together with a common desire to work collectively to improve the quality of care and outcomes for their patients. This is done through a programme of activities to support our member practices and working with partners in the wider health and social care system to support the needs of our patients.

About us

Camden Health Evolution (CHE) is a limited company set up by our member practices. The Board of Directors are composed of a Chief Operating Officer, a Finance Director and two PCN Clinical Directors. The shareholders are made up of GPs from our member practices.

CHE is a Federation of 9 Camden General Practices that together provide care for a population of 85,409 patients.

Our practices all provide NHS services and as an organisation wholly owned and led by GPs, we are committed to the founding principles of the NHS and passionate about the continued sustainability of General Practice.

Our key priorities are:

  • To support autonomous, equitable, high quality & sustainable general practice.
  • To improve health outcomes, reduce health inequalities & ensure effective use of resource to the residents of Camden.
  • To provide strong leadership and a collective voice for primary care within the wider health & social care system.
  • To be a leading partner in the development of integrated care networks with local patients & providers and to ensure that these changes meet the needs of our patients.
  • To support our Primary Care Network (PCN) & member practices through infrastructure support.
